  REGISTRATION FEE: $65/one child, $100/two children, $125/three or more children. There is an additional fee of $25 for any student receiving the sacraments of First Communion and Confirmation. Please make checks payable to St. Michael's Parish Faith Formation. (If this presents a hardship, please let us know).

- I give permission for my child/children to attend religious education classes and other components of the curriculum and, if needed, to be evaluated, diagnosed, treated, and/or medicated in accordance with standard medical practices by licensed medical personnel.
  
  I relieve the parishes of St. Michael the Archangel and St. Jude the Apostle of all responsibility and consequences that may arise as a result of this treatment. I will not hold St. Michael's or St. Jude's Churches, its volunteers, or its representatives responsible in the event of an injury.

- BEHAVIOR/DISCIPLINE POLICY
- Catechesis, the sharing of faith, takes place every moment of every day starting with your own family. Our catechists share their faith and life experiences as volunteers. They are limited in what they can accomplish in the very short time that they have with your children. Time spent on discipline issues is time taken away from religious education. We expect every child to behave in a respectful manner with an attitude of openness to the faith experience.
  Cell Phones must be OFF and OUT OF SIGHT during Faith Formation time. If a child has a phone out during class time, they will be asked to give it to the catechist until the end of class. If it happens again, the phone will be held until a parent comes to pick it up. Classes are only a short period of time, and we are working to be sure that prayer and learning are not disturbed.
